About dr. primero ng
Dr. Primero Ng
Dr. Primero Ng, MBBS (Hons), FRACP, FACC, FSCAI, is a Consultant Interventional Cardiologist based in Perth, Western Australia, with advanced expertise in complex coronary artery disease. His practice focuses on the treatment of chronic total occlusions, high-risk and complex percutaneous coronary intervention, and patients who have been declined or are unwilling to undergo coronary artery bypass surgery. He is also dual-trained in Cardiac CT, providing an imaging-led approach to diagnosis, procedural planning, and follow-up care.
Dr. Ng graduated with honours from the University of Western Australia and completed his specialist cardiology training across major tertiary hospitals in Perth. He subsequently undertook advanced coronary training at Royal Perth Hospital, where he developed a strong foundation in intravascular imaging and coronary physiology. These tools, including intravascular ultrasound, optical coherence tomography, and pressure-wire assessment, are routinely used in his practice to enhance procedural precision, safety, and long-term outcomes.
Dr. Ng was later selected as the only overseas fellow to train at the University of Washington Medical Center in Seattle, internationally recognised as one of the world’s leading centres for complex coronary intervention and chronic total occlusion PCI. There, he completed a formal Complex and High-Risk Indicated PCI fellowship under the mentorship of global leaders in the field, including Dr. Bill Lombardi, Dr. Jamie McCabe, Dr. Kate Kearney, and Dr. Lorenzo Azzalini. This highly competitive fellowship emphasised not only technical excellence but also leadership, culture-building, and an unwavering commitment to patient safety and outcomes.
Dr. Ng’s clinical expertise spans the full spectrum of complex coronary intervention, including long-standing complete blockages, heavily calcified arteries, multivessel and left main coronary disease, and revascularisation strategies for patients with limited treatment options. His approach is data-driven, imaging-guided, and carefully tailored to each patient’s anatomy, symptoms, and goals.
Dr. Ng currently consults and sees patients in private practice at the Mount Hospital and in Mandurah, while also holding a public appointment as an Interventional Cardiologist at Royal Perth Hospital. At RPH, he is the Co-Lead of the Complex Coronary Therapeutics Program, where he is actively involved in advancing the delivery of complex coronary care, developing structured pathways, and improving access to advanced interventional techniques for patients across Western Australia.
With a strong focus on quality of life, Dr. Ng believes that successful cardiology care extends beyond technical success. Through world-class training, collaborative practice, and thoughtful decision-making, he is committed to delivering high-quality, evidence-based coronary care across Western Australia, offering clarity and durable solutions where options may appear limited.

Patient Education
Understanding Complex Coronary Disease & Chronic Total Occlusions (CTO)
What is a chronic total occlusion (CTO)?
A CTO is a coronary artery that has been completely blocked for several months or longer. Blood often reaches the heart muscle through small natural bypass vessels, which is why symptoms can be subtle or progressive rather than sudden and dramatic.

Did You Know? Up to 20% of patients have a CTO identified during coronary angiography, yet many remain untreated despite ongoing symptoms.
Common Symptoms
Symptoms vary from person to person and may include:
  • Chest discomfort or tightness
  • Shortness of breath with activity
  • Fatigue or reduced exercise tolerance
  • Symptoms that limit work, daily activities, or enjoyment of life
Some patients are told their symptoms are "manageable" or that nothing more can be done.
That is not always true.
Why Treatment Can Help
The primary goal of treating complex coronary disease and CTOs is to improve quality of life.
For carefully selected patients, treatment can:
  • Reduce or eliminate angina completely
  • Improve exercise capacity and energy levels
  • Restore confidence in daily activities
  • Reduce reliance on multiple medications
  • Improve overall patient wellbeing and happiness
Treatment is not about chasing perfect looking arteries.
It is about helping you live better.

A Patient-First Approach
Not every blockage needs a procedure. Not every patient benefits from intervention. A patient-first approach means understanding how your symptoms affect your life, carefully reviewing scans and test results, discussing all options including medical therapy and procedures, and recommending treatment only when the potential benefit is clear.
Modern Techniques, Thoughtful Care
Advances in imaging and catheter-based techniques now allow many complex blockages to be treated safely and effectively. These procedures require careful planning, experience, and a strong focus on safety. When intervention is recommended, the priority is durable symptom relief with the lowest possible risk.
"Your symptoms matter. Your quality of life matters. That is always the priority."
Advanced Care
Advanced Interventional Techniques
What Does "Advanced" Mean
Advanced coronary care is about more than treating blockages. While some heart artery narrowings are straightforward, others are more complex and require specialised expertise.
Advanced interventional techniques use modern, minimally invasive approaches to treat difficult or long-standing coronary blockages safely and effectively, often without the need for open-heart surgery, with the goal of improving symptoms and quality of life.
Why Experience Matters
Complex coronary procedures are not about using one technique. They require the ability to select and combine different approaches depending on the anatomy, the blockage, and the patient.
Mastery means knowing:
  • Which technique is most appropriate
  • When to change strategy mid-procedure
  • When not to proceed
  • How to manage challenges and complications safely
This level of decision-making comes from dedicated training and high procedural volume.
A Full Toolkit, Used Thoughtfully
Dr. Ng is trained in and experienced with the complete range of advanced coronary techniques, including:
Advanced Wire Techniques
Modern wire and catheter techniques to cross difficult blockages that others may find impossible to treat.
CTO Recanalization
Specialized techniques to reopen long-standing complete blockages using contemporary approaches.
Calcified Artery Treatment
Strategies for heavily calcified or previously stented arteries that require expert management.
Imaging-Guided Precision
Advanced imaging to guide precise stent placement and optimize procedural outcomes.
Safety-First Methods
Methods to improve safety and protect heart muscle during complex procedures.

Precision Guided by Intravascular Imaging
Advanced imaging using intravascular ultrasound (IVUS) and optical coherence tomography (OCT) from inside the artery improves precision, safety, and long-term outcomes during complex heart procedures. Although not widely used due to the expertise required, imaging-guided treatment is central to modern complex coronary care.
Safety Always Comes First
Not every blockage should be treated, and not every procedure should be pushed to completion. A mastery-based approach prioritizes careful planning, clear stop points, calm management of complications, and long-term benefit over technical success alone.
What This Means for You
For patients, advanced techniques are not about complexity for its own sake. They are about greater chance of symptom relief, fewer repeat procedures, safer treatment of difficult blockages, and better quality of life overall.
Your Journey
The Patient Journey: From Diagnosis to Recovery
Every patient's journey is unique, but the commitment to excellent care remains constant at every stage.
1
Understanding Your Symptoms
Your journey begins with listening. Symptoms such as chest discomfort, shortness of breath, or reduced exercise tolerance are explored in the context of your daily life, not just test results.
2
Accurate Diagnosis
Appropriate investigations are reviewed carefully, which may include scans, stress tests, or coronary imaging. The goal is to understand what is causing your symptoms and whether your heart arteries are contributing. Not every blockage needs treatment.
3
Discussing Your Options
All treatment options are discussed clearly and honestly, including optimised medical therapy, lifestyle and risk factor management, and interventional procedures when appropriate. Decisions are shared, informed, and aligned with your goals.
4
Care in the Right Setting
If a procedure is recommended, care can be provided in either public or private hospital settings, depending on what is most appropriate for you. Planning is meticulous, and safety is always the priority.
5
The Procedure
Procedures are minimally invasive and guided by advanced imaging to maximise accuracy and safety. Every step is planned in advance, with clear strategies and stop points. The aim is meaningful symptom improvement, not technical complexity.
6
Recovery and Follow-Up
Recovery focuses on getting you back to daily life as smoothly and safely as possible. Follow-up includes review of symptoms and recovery, medication optimisation, and ongoing support and education. You are not discharged from care and left on your own.
7
Long-Term Quality of Life
The journey does not end with a procedure. Ongoing care is centred on sustained symptom relief, confidence in physical activity, and long-term heart health. The ultimate goal is a better quality of life.
